Regarding Orders:

Minimum Order Quantity:	501 pieces on terminal insulators 50 pieces on electro-mechanical items
Lead Time:	Insulators: 15 weeks Electro-Mechanical: 6 weeks
Rush Ship Date:	TERMINAL INSULATORS Please contact FSI for setup and expedited delivery charges. ELECTRO-MECHANICAL Please contact FSI for setup and expedited delivery charges.

Payment Options:	American Express, Discover, Mastercard, Visa
Terms:	1% - 10 Net 30 (freight prepaid and added) 2% - 10 Net 30 (billed to your UPS account) * * Applies to accounts in good standing only
To apply for a Terms: Net 30 billing account, please mail a signed printout of this form to FSI.

Note 1:
An EMAIL acknowledgement will be sent to you after your order has been entered. This acknowledgement will allow you to verify that your order has been placed correctly, before we start processing. Please respond to this EMAIL that you agree with the information in the acknowledgement.

If we do not receive a response to the acknowledgement within one (1) business day, FSI will assume the order has been placed correctly and place your order in the queue.

Blanket Orders

FSI will accept blanket orders allowing multiple releases for a period of up to 9 months.

Example: 5 releases of 1000 pieces for a 5000 piece price.

To accomodate the wide number of variables ( quantity, release dates, etc ), please contact FSI directly and speak with a customer service representative to devise a blanket plan.

Regarding Shipping Times:

All orders close on the day of shipment at 12:00 NOON EST.
